Arrival Time

Please arrive 15 minutes before your scheduled appointment to complete a questionnaire/registration form. You are welcome to bring someone with you to your appointment.

What to Bring

Federal and state regulations require that we verify your identity with photo identification and check your address. Patients over the age of 18 will be asked to show the following forms of identification:

Photo ID (i.e. driver’s license, passport)

Identification card that verifies your address (i.e. driver’s license)

Health insurance card (ie; United Health Care insurance) if insured

We also recommend that you carry a list of your current medications, dose and frequency. If you’re a new patient, please bring your previous doctor’s information (name, address, phone number). Check with your insurance company, as you may be required to pay your deductible or co-payment.

Late Arrivals 

If you arrive more than 15 minutes late, you may have to wait until the doctor has seen other patients who were on time. We make every effort to try and accommodate all of our patients, however, you may be asked to reschedule your appointment for another day.

Medical Emergency

Please call your physician directly and leave a message. If you are unable to reach them and the situation is urgent please go to the nearest emergency room and try and page them again.


If you need to cancel your appointment, please call the office at 301-468-1001 as soon as possible but no less than 48 hours in advance to reschedule. We try to fill our cancelled appointments with other patients but if we are unable to do so we may have to charge you the full fees for your visit if you cancel less than 48 hours before your scheduled time.

Prescription Refills Policy

For prescription refills either call your pharmacy and ask them to fax us a refill request at 301-468-1101 or call the front desk at 301-468-1001. It may take up to 48 hours to refill prescriptions. For any urgent needs please leave us a message or call your doctor.

Medical Records Requests (Release of Information)

To receive a copy of your medical records, you must submit a request in writing.

You may return the completed form to your physician’s office by mail, fax, or in person. Please allow a week for processing of routine medical records requests. There may be a charge for record releases.
